Quotes left.png I have difficulty keeping my sanity. Each day, this altering world provides me with new trials. A man wearing the robes of a doctor crossed my path. From afar, I witness him walking around, searching.. But he was not...normal. Eyes and mouth pried open, in a painful and disturbing way. Where do these fiends stem from? I do not trust him, he looks like he wants to bring pain, not healing? I will stay away, it is the wisest thing to do.

Lore

Original Lore

This is the original Lore of The Doctor as it was presented in the official Game Manual on Dead by Daylight's original website (no longer accessible).

From an early age, Herman Carter understood the human psyche. To analyse and de-construct something as powerful as the brain intrigued him. He was an apt pupil and gained the attention of his teachers. He excelled in high-school and was published in "Partisan" — a psychology gazette. Within a year, Carter was fast-tracked into Yale's advanced neuroscience programme, really a front for the CIA. Brainpower is a must if you're about to conquer the world and demolish foes across the pond. The CIA understood this, so interrogation and intelligence became their number one priority. All they needed were brilliant people — like Carter.

Carter and other top-tier recruits were transferred off-campus and into a secret black site facility in Illinois known as Léry's Memorial Institute. A protégé craves a mentor, and that's where Dr Otto Stamper stepped in, who taught Carter that information is everything and knowledge is power. He was given all instruments needed, a guiding hand, and more or less everything he asked for. He never realised that sunlight had started to become so scarce, that he too was kept in the dark. Because knowledge doesn't only give you power, it also transforms you into a threat.

To extract information was his mission. Dr Stamper encouraged Carter to go further and not to consider this a normal medical facility — no eyes were watching them, there were no rules to abide. The agency just pointed Carter in the right direction, then he started to take a few steps back as he saw how Carter could walk on his own. Docile test subjects were exchanged for real, live spies. People that played a role in the troubles outside the facility. Carter shouldered this new role — Project Awakening took form, and on paper Carter described it as "experimental interrogation". It was approved and over a few months, nobody knocked on his door. Screams and moans filled the corridor outside his lab, but wars skew people and what they accept — as long as the enemy is kept at bay. The fluorescent lights flickered more and more often. Electro-convulsive Treatment became a standard dish on the menu. Prisoners held at the facility begged the guards to take them to any other lab but Carter's. Rumours were disregarded in the beginning.

Over the years, Carter became known as The Doctor and no one ever questioned if he had even held a medical certificate or even what happened to the prisoners after they had given up their information. It was only after the Léry's Memorial Institute went silent for a week that they finally uncovered the true horror of what had happened there.

Carter's experimental information extraction had turned to horrific and bizarre torture. Patients and prisoners were found dead or in vegetative states with all types of head trauma. In his office, they found the most terrible discovery of all. Dr Stamper himself, his head peeled open and an array of electrodes and sensors inserted into his still working, but annihilated brain. There was no sign of Herman "The Doctor" Carter, but his research papers suggested that he had been using the prisoners as part of awful ECT experiments as he searched for the panacea of mind control.

The government didn't want to know. The black site was condemned and all knowledge of the Léry's Memorial Institute redacted forever.

Revised Lore

This is the revised Lore that is currently featured in the Game.

Showing exceptional aptitude in psychology, Herman was handpicked for special training in an advanced neuroscience program located in a secret black site facility in Illinois known as Léry's Memorial Institute, really a front for the CIA.

This is where Herman met Dr Otto Stamper, where, under his guidance, Herman began to use strange and ever increasingly vicious methods to extract information from prisoners sent to the institute, which was also a covert prison and re-education centre for whoever was the USA's current nemesis. His liberal use of violent electroshock therapy revealed dramatic results and several threats to national security were uncovered.

Over the years, Herman became known as The Doctor and no one ever questioned if he had even held a medical certificate or even what happened to the prisoners after they had given up their information. It was only after Léry's Memorial Institute went silent for a week that the true horror was finally discovered. The personnel, patients, and prisoners were all found dead with all types of head trauma. All personnel and prisoners' bodies were accounted for, including Dr Stamper, but no sign of Herman “The Doctor” Carter.

SPECIAL ABILITY: CARTER'S SPARK
Striking Survivors with his electro-convulsive abilities, The Doctor causes them to suffer from the tiered MadnessIconStatusEffects madness.pngIconStatusEffects madness.pngIconStatusEffects madness.png Status Effect.

SPECIAL AFFLICTION: MADNESS

    • Unhooking other Survivors is exempt from this restriction.

SPECIAL INTERACTION: SNAP OUT OF IT
Survivors having reached Madness III can perform the Snap Out of It interaction for 12 seconds:

  • Triggers Madness Skill Checks at each quadrant of the Progress bar.
  • Succeeding them continues the interaction.
  • Failing them has the following repercussions:
    • Interrupts the action and resets its Progress to the previous quadrant.
    • Causes the Survivor to scream and reveal their location.
  • Completing the interaction resets Madness to Tier I.

Alternate Character Models

The Doctor has 2 alternate Character models available in the in-game Store:

DLC

Main article: Downloadable Content

The Doctor is part of 3 DLCs:

Change Log

Hotfix 1.5.1a

  • Nerf: removed the (unintentional) ability to automatically shock injured Survivors anywhere within The Doctor's Terror Radius when performing a Shock Therapy attack.

Hotfix 1.5.1b

  • Buff: increased Treatment Mode Movement speed from 4.3 m/s to 4.4 m/s.
  • Buff: increased the Shock Therapy Attack range from 7.5 metres to 9 metres.
  • Buff: adjusted the Shock Therapy Area of Effect to also hit Survivors who, in terms of elevation, are slightly above or below The Doctor.
  • Buff: increased Snap Out of It duration by 4 seconds to 12 seconds.
  • QoL: added the ability to read the Aura of Illusionary Doctors when the afflicted Survivor has Tier III Madness to The Doctor's base-kit.
  • Change: Hallucination Aura colour changed from "Notice Me!" Hot Pink to "I Indicate An Ally" Yellow.

Patch 1.6.2

For additional information about this special update, see Doctor Cube.
  • Buff: Snap Out of It now has a chance to trigger Skill Checks.
    • Failing those Skill Checks will interrupt the action and cause the Survivor to scream, revealing their location.
  • Buff: added Madness Skill Checks to the base-kit, allowing them to affect all skilful interaction rather than just Snap Out of It.
  • Buff: removed the Movement Speed penalty when switching Stances; The Doctor now already moves at Treatment mode speed.
  • Buff: added an interaction cool-down of 2.5 seconds to Survivors hit by the Shock Therapy attack.
    • This change also added a new icon to the HUD for Survivors: whenever they are shocked by Shock Therapy, The Doctor's Power icon in the red debuff colour is displayed with a timer overlaid on top of it.
While the icon is visible, interactions are inhibited for the afflicted Survivor.
  • Buff: added the ability for Tier III Madness to prevent Survivors from using Med-Kits and Toolboxes, as well as healing themselves, other Survivors, or repairing Generators.
  • Change: Snap Out of It is now considered a Healing action.
    • This change is deemed neutral, as it had both positive and negative effects for The Doctor, with Perks like A Nurse's CallingIconPerks aNursesCalling.pngIconPerks aNursesCalling.pngIconPerks aNursesCalling.png revealing the Aura of Survivors, but Perks like Botany KnowledgeIconPerks botanyKnowledge.pngIconPerks botanyKnowledge.pngIconPerks botanyKnowledge.png also speeding up the interaction.

Patch 1.9.2

  • Buff: extended the ability for Tier III Madness to prevent Survivor interactions to include Sabotage actions as well as the use of any Item.

Patch 2.0.0

  • QoL: changed Illusionary Doctors to always wear the same Outfit as The Doctor himself, instead of his default one.

Patch 2.4.0

Patch 3.5.0

  • Rework:
    • Buff: increased the Shock Therapy range from 9 metres to 10 metres (the actual new value was 10.7 metres).
    • Buff: removed the need to switch Stances in order to use Shock Therapy.
    • Buff: replaced the Static Field with the Static Blast ability.
    • QoL: Illusionary Doctors now use The Doctor's idle stance instead of his menu stance.
    • Nerf: halved the Decay rate of Madness while outside of the Terror Radius from -1 p/s to -0.5 p/s.
    • Change: adjusted the modifier of his Movement speed while charging Shock Therapy from 70 % to 67 % to keep the same Movement speed of 3.08 m/s he had in Treatment Mode.
  • Rework: reworked the Snap Out of It interaction.
  • Change: The Doctor now features a unique Menu Music, Terror Radius and Chase Music.

Patch 3.6.0

  • Buff: removed the ability for Madness to decay over time.
  • Buff: added the reversed variant of Madness Skill Checks to the base-kit.
  • QoL: standardised the Madness Tiers, with Shock Therapy filling 50 % and Static Blast 100 % of a Tier.

Patch 4.7.0

  • Quality of Life: Madness Tiers now each possess a distinct visual on the HUD.
  • Quality of Life: improved The Doctor's visual effects to improve performance.
    • Reduced the intensity of the "Glitch" and "Madness" screen effects.
    • Removed the visual horror effect showing The Doctor's face after failed Madness Skill Checks during Snap Out of It.
    • Updated and increased level-of-detail of his character model and various cosmetics.

Patch 7.6.0

  • Buff: increased the range of the Shock Therapy attack from 10.7 metres to 12 metres.
  • Buff: reduced the Detonation delay of Shock Therapy from 1 second to 0.8 seconds.

Patch 8.2.0

  • Buff: reduced Static Blast's default cool-down from 60 seconds to 45 seconds.
  • Buff: Static Blast's cool-down is now further reduced to 30 seconds if no Survivors are in range.
  • Buff: increased the Movement Speed while charging Static Blast from 1.16 m/s to 2.99 m/s.

Patch 8.6.0

  • Reinstation: reinstated the visual horror effect on failed Madness Skill Checks during Snap Out of It (removed in Patch 4.7.0), though slightly modified.
    • Instead of showing The Doctor's entire face, it now shows various zoomed-in parts of it.
  • Indirect Nerf: Mending now pauses while performing Snap Out of It.
    • This change prevents Survivors suffering from Deep Wound and Madness III at the same time from essentially being forced to enter the Dying State once their Bleed-out timer depleted past the point of matching the duration of the Snap Out of It interaction, which takes precedence over Mending.

Patch 8.7.0

  • Nerf: added a cooldown to interrupting Unhooks with Shock Therapy repeatedly.

Trivia

  • The Doctor introduced the MadnessIconStatusEffects madness.pngIconStatusEffects madness.pngIconStatusEffects madness.png Status Effect and Game Mechanic, which is unique to Trials featuring him.
  • The Doctor was the first Killer to have had two different stances, Punishment Mode for regular attacks and Treatment Mode to use his Power.
    • Treatment Mode allowed The Doctor to perform grabs, but prohibited any attacks with his primary weapon.
      • Since so-called Treatment Grabs (i.e. spamming the Power during a Hook Stand-off) were virtually guaranteed to succeed (since in this mode, it counted as a grab attempt without triggering a Basic Attack), it made unhooking against a face-camping Doctor near impossible to counter.
  • The Doctor's Memento Mori animation was the first one in which Survivors saw part of the animation from the Killer's perspective.
  • Due to technical limitations, Illusionary Doctors originally used to wear the default customisation option of The Doctor, making them easily distinguishable from Doctors wearing either the Prestige or Project Awakening outfits.
    • This was changed with Patch 2.0.0: Illusionary Doctors now wear the same customisation options as The Doctor.
  • After receiving a rework with Patch 3.5.0, The Doctor received his own Main Menu Theme and unique Chase Music.

Lore Trivia

  • The Doctor's facial equipment is a reference to the 1971 Movie A Clockwork Orange where the protagonist undergoes brutal psychotherapy treatment in an attempt to cure his mania and bloodlust.
  • The Doctor's background story may be a reference to MKULTRA, a CIA research project that officially ran from 1951 to 1973 and experimented with unethical methods of interrogation and mind control.
  • "Herman" was a widely popular name in the US in the 19th century and remained so throughout the first half of the 20th.
  • "Carter" is a surname common among African Americans capable of tracing their roots back to the southern United States or Caribbean from the early 20th century onward.
  • Some file names reveal that during development, The Doctor was originally called "Mu Yi", a name that sees The Doctor's likely original nationality as either Chinese or Korean.
    • The terms "Mu" (無 in Japanese; 무 in Korean) or "Wú" (无 in Chinese) mean "not have; without" and are a key word in Buddhism, especially in Zen traditions.
    • The term "Yi" (義 in Confucianism) means "justice, righteousness, meaning", a moral disposition to do good.
    • When combined, Carter's original name literally translates to either "without justice" or "without meaning".
  • Other file and WIP names of his Add-ons ("Jade Charm", "Nanmu Catapult", "Tiger Fang Charm") also reveal that The Doctor was generally supposed to be more Chinese-themed at an early point in development.
    • This was later changed to his current name and ancestral descent.

Voice Actor

  • The Doctor is voiced by Alex Lin, who at that time worked as a Product Manager on the Game.

Cosmetic Trivia

The Doctor as "The Look-See"
  • The Doctor's Legendary Outfit, "The Look-See", is a Character from the Crypt TV series of the same name.
    • "The Look-See" is an evil entity who haunts those unable to let go of their grief.
After leaving a cryptic note as a warning of its imminent appearance, "The Look-See" murders its victim and takes a body part symbolic of the reason for the victim's grief.
  • Whether the naming/design was a mistake or intentional, The Doctor's weapon in the Very Rare "Pestilence Physician" cosmetic, Conductive Caduceus, was incorrectly named and designed unfittingly for The Doctor's lore.
    • The caduceus is the staff of Hermes, the Greek herald of the gods, which has two snakes. The rod of Asclepius (also called the asklepian), the Greek god of medicine, has one snake. Hermes' caduceus is often inappropriately misinterpreted as a symbol of medicine because of its association with thieves and liars, to which Hermes is a patron of.
    • The Doctor's lore focuses on his pursuit of discoveries and knowledge in medicine, which fits perfectly with the asklepian of Asclepius rather than the caduceus of Hermes.
